Audiogram
-
Pulmonary
Function Test
-
PPD

-
Hepatitis

exposure to bloodborne pathogens)
-
Blood

metals, etc.)
Employers
requiring heavy lifting, frequent bending, lifting,
carrying, climbing, and other strenuous physical tasks, may
add a functional work capability screen to the basic
physical exam.
Return To Work Evaluations
Return to Work Evaluations are tailored to meet the specific
needs of both the employer and the employee. Employers are
encouraged to provide a job description that outlines the
essential functions of the position to assist the provider
in assessing the ability of the individual to safely return

Respirator Evaluations
Employees required to have medical clearance to wear a
respirator will first complete the OSHA medical
questionnaire. A medical provider will review that
questionnaire to determine if that employee is able to wear
a respirator or if further evaluation is necessary.
